What is Home Health Care?

Home Health Care is a critical part of healthcare today, touching the lives of millions. It represents a wide range of professional healthcare assistance and support services in the home. While hospital stays have decreased, the number of people who require skilled services in the home has increased significantly.

As a clinically owned and operated organization, it is our goal to provide excellent quality care, while having a positive impact on the lives of our patients, their families, and our healthcare partners throughout the continuum of care. Our Home Health Services will utilize all available resources to provide the highest level of care possible to our patients in the optimal healing environment, the home.

Home Health Services include healthcare management and clinical experts that will provide uncompromising levels of attention, customer service and integrity.

Our team of licensed clinicians will work with patients and physicians to develop a plan of care that will meet each individual’s specific needs. Medicare and Medicaid are accepted. Skilled
Registered Nurses, (RNs), perform patient evaluations, including monitoring and tracking vital signs, clinical procedures such as IV placement, phlebotomy, and administering medications.

In the home, your nurse will monitor your progress and keep in contact with your physician to make sure that your treatment protocols are appropriate. We are proud to offer multilingual services including Spanish, Russian, Farsi, and Lithuanian as well as language line translation services.
